Most squad failures with customer-facing executors are not configuration bugs. They are **breadth failures**: the executor was prompted with a literal role label ("pharmacy attendant — sells medicine") and reproduces that clipped world model verbatim. A real pharmacy sells candy, snacks, cosmetics, vitamins, baby products, gum, condoms, gift cards, and lottery tickets — sometimes 30%+ of revenue. When the customer asks for candy and the executor says "we only sell medicine", the squad is dead in the water on its first non-obvious turn.
