+## Issues Encountered
+
+1. **ECHO Law 1 violation:** Orchestrator assumed FID-082 was not implemented without checking codebase. User caught this.
+2. **ContextCompactor overwrite:** `write_file` was called with wrong path, destroying the ~350-line ContextCompactor
+   class. Reconstructed from usage patterns in run-agent-step.ts.
+3. **Verifier tool availability:** Verifier agent got stuck trying to use `transition_phase` and `read_files` (it has no
+   tools by design).
+4. **Basher phase restrictions:** Bashers couldn't run `run_terminal_command` in self_correct phase. Switched to `run_readonly_command`.
+5. **Scribe deadlock:** Scribe couldn't write files in idle phase. Resolved by user ending turn.
+
+## Lessons Learned
+
+1. FID status is a claim, not ground truth — always verify against codebase
+2. Law 1 applies to status reporting, not just code edits
+3. Word-boundary regex (`\bGOAL_SATISFIED\b`) is more robust than `includes()`
+4. Static imports preferred over dynamic `import()` for synchronous operations
+5. `useState` must be at top of file with other React imports
+6. ContextCompactor reconstruction from usage patterns is possible but risky — always backup critical files

+## What We Found (Root Cause Analysis)
+
+The `/history` command shows incomplete sessions. Four layered issues:
+
+### Issue 1: DB/Filesystem Persistence Split (PRIMARY)
+
+**Files:** `cli/src/utils/run-state-storage.ts`, `cli/src/hooks/use-send-message.ts`
+
+Two write paths exist:
+

+**The bug:** `loadMostRecentChatState()` (line ~150) tries DB first. If DB returns a result, it returns immediately —
+never checking the filesystem for a newer checkpoint. Messages saved only via async checkpoints (mid-stream state) are
+silently discarded on reload.
+
+**The fix:** `saveChatStateAsync` must also update the DB (throttled, e.g. every 30s). AND/OR `loadMostRecentChatState`
+must compare DB timestamp vs filesystem timestamp and prefer the newer one.
+
+### Issue 2: Dual-Source History Listing
+
+**File:** `cli/src/utils/chat-history.ts`
+
+`getAllChats()` merges results from:
+
+- `getAllChatsFromDb()` — reads all chat IDs from SQLite
+- `getAllChatsFromDisk()` — scans `chats/` dir for `chat-meta.json`
+
+These can diverge. A DB entry may exist for a chat whose FS data was lost, or vice versa → ghost sessions (0 messages)
+or missing sessions.
+
+### Issue 3: No Session-Completeness Marker
+
+**File:** `cli/src/utils/chat-meta.ts`
+
+`ChatMeta` type has `title`, `timestamp`, `messageCount`, `model` — but no `completed: boolean`. Users can't distinguish
+a graceful session end from a crash.
+
+### Issue 4: Sidecar Staleness Window
+
+**File:** `cli/src/utils/chat-meta.ts`
+
+`writeChatMeta` records `messagesSize`/`messagesMtimeMs` for staleness detection. But there's a window between
+`chat-messages.json` write and `chat-meta.json` write where concurrent reads get stale data.

+### Step 1: Add DB write to async checkpoint (run-state-storage.ts)
+
+In `saveChatStateAsync`, after writing to filesystem, also update the DB. Throttle DB writes to avoid excessive SQLite I/O:

+Ensure `chat-meta.json` is written atomically (write to temp, then rename) AFTER `chat-messages.json` is fully flushed.

+Before reading from the store, verify the session is fully loaded. If not, load from disk first.
+
