+### Evidence and Findings
+
+#### LLM-1 — CRITICAL — Chat `doGenerate` dereferences `choices[0]` without a guard
+
+**File:** `packages/llm-providers/src/openai-compatible/chat/openai-compatible-chat-language-model.ts:242-263`
+

+The response schema (`OpenAICompatibleChatResponseSchema`) declares `choices: z.array(...)` with **no minimum length**,
+so `choices: []` is a valid provider response (commonly returned for content-filtered or degenerate completions).
+`choice` is `undefined` → `choice.message` throws `TypeError: Cannot read properties of undefined`.
+
+**Why CRITICAL:** the schema permits the empty array, the crash takes out the primary chat hot path with no
+workaround, and FID-006 rated the identical completion-model bug CRITICAL (LLM1) — this is the same defect class
+left unfixed in the more-traveled path.
+
+FID-006 LLM1 fixed the **completion** model (`openai-compatible-completion-language-model.ts:193-196` uses
+`choice?.text`, with a regression test at `completion-language-model.test.ts:56`), but the **chat** model — the primary
+hot path used by every chat provider — was missed. There is **no** empty-`choices` test for the chat model (grep for
+`empty choices` in `openai-compatible-chat-language-model.test.ts`: none).
+
+**Fix:** guard with `choice?.` (`const text = choice?.message.content`) or early-return an empty-content result, and add
+a regression test mirroring the completion one.
+
+#### LLM-2 — MEDIUM — Completion `getArgs` spreads raw provider options onto the wire unfiltered
+

+`logitBias` (raw) on the wire. Inconsistent with the chat model; can trigger strict-provider 400s and leaks internal
+option names. **Fix:** apply the same schema-key filter as the chat model, or stop mapping and rely on the raw spread.
+
+#### LLM-3 — LOW — `z.any()` in the shared error schema
+

+#### LLM-6 — LOW — Stream error-chunk payload shape differs between chat and completion
+

+Consumers of `LanguageModelV2StreamPart['error']` get a string in one path and an object in the other. **Fix:** unify
+the payload (and type it).
+
+#### LLM-7 — LOW — `parseToolCallArguments` has a no-op conditional
+

+
+`stream-transform.test.ts` declares `transform(chunk: any, controller: any)` and `const emitted: any[] = []`. Test-only,
+so Law 6 impact is limited, but typed helpers would keep the tests honest as the stream API evolves. **Fix:** type the
+test doubles with the real `TransformStream` generics.
+
+#### DB-1 — MEDIUM — Unguarded import-time DB open + schema DDL
+

+All of this runs at module import. If `echo.db` is corrupt, unwritable, or `SAVANT_DB_PATH` points into a
+non-existent directory, the **entire CLI crashes at startup** with no graceful degradation path (the CLI's
+`db-storage.ts` wraps reads in try/catch, but the module-level open is unguarded). **Fix:** guard the open + schema
+init (try/catch → fail-open with a logged fallback, or a dedicated `initDatabase()` the CLI calls deliberately).
+
+#### DB-2 — MEDIUM — Message/session ordering relies on second-granularity `created_at` ties
+
+`getMessagesBySessionId` (`service.ts:311`) orders by `created_at ASC` only; `getSessionsByChatId` (:115),
+`getLatestModel` (:161/:164), and `saveModel`'s latest-session path (:153) all order by `created_at` alone.
+`created_at` is `DEFAULT CURRENT_TIMESTAMP` — second granularity. Messages persisted within the same second (common
+during streaming saves) have **no guaranteed order**; message replay order feeds LLM context. **Fix:** add a
+deterministic tiebreaker (`ORDER BY created_at, rowid`) to the ordering queries.
+
+#### DB-3 — MEDIUM — Three overlapping model-attribution APIs with divergent semantics
+
+- `updateSessionModel(sessionId, model)` — targets one session (`service.ts:124`).
+- `saveModel(model, chatId?)` — with `chatId` updates **all** sessions for that chat; without, updates the latest
+  session by `created_at` (`:143-155`).
+- `getLatestModelForChat(chatId)` (`:133`) duplicates `getLatestModel(chatId)` (`:158-162`) exactly — and has **zero
+  callers** (dead code).
+
+The `saveModel` non-chatId fallback is also non-deterministic under the DB-2 tie issue. **Fix:** collapse to one
+read + one write path; remove `getLatestModelForChat` (Law 13).
+
+#### DB-4 — LOW — Dead exports
+
+`hasSessions` (`service.ts:172`), `createAgentConfig`/`getAgentConfig` (`:218/:232`), `getCostRecord` (`:336`),
+`getDatabase` (`index.ts:129`), `closeDatabase` (`index.ts:133`) have zero callers outside the package (verified by
+repo-wide grep; `local-agent-registry.ts` imports only template CRUD). `closeDatabase` being uncalled is benign for a
+CLI process, but the exports are dead surface. **Fix:** remove or wire them (Law 10/13).
+
+#### DB-5 — LOW — Non-null assertions on get-after-create round trips
+
+`createSession` returns `getSession(id)!`, `createCostRecord` returns `getCostRecord(id)!`, `createFidDocument`
+returns `getFidDocument(fidId)!`, etc. If the INSERT silently failed (e.g. FK violation), `get*` returns `null` and the
+`!` produces a confusing runtime TypeError instead of a clean error. **Fix:** propagate an explicit error on
+null-after-create.
+
+#### DB-6 — LOW — UPDATEs return void with no affected-rows check
+
+`updateSession`, `updateSessionModel`, `updateAgentTemplate`, `updateFidDocument` silently no-op on a missing id.
+**Fix:** return `changes` from `stmt.run()` (`result.changes > 0`) so callers can detect misses (the CLI already
+round-trips FID existence, so impact is low).
+
+#### DB-7 — LOW — Image model does not enforce `maxImagesPerCall`
+
+`OpenAICompatibleImageModel` exposes `readonly maxImagesPerCall = 10` but `doGenerate` passes `n` through with no
+clamp/validation. The embedding model throws `TooManyEmbeddingValuesForCallError` for the same class of issue;
+the image model should too. **Fix:** validate `n <= maxImagesPerCall`.
+
+#### DB-8 — LOW — `VERSION` falls back to `'0.0.0-test'`
+
+`openai-compatible/version.ts` uses the Bun compile-time define `__PACKAGE_VERSION__` with a `'0.0.0-test'` fallback.
+Verify the build pipeline injects the real version into the User-Agent suffix; otherwise every request advertises
+`0.0.0-test`. **Fix/verify:** confirm injection in the CLI/agent-runtime build, or drop the suffix.
+

+
+The llm-providers package is a fork/adaptation of the AI SDK's openai-compatible provider, and FID-006's hardening
+(LLM1 empty-choices guard, LLM2 model re-assertion, LLM3/LLM4 option parsing) was applied incrementally and
+incompletely — the chat model, being the most-copied original, kept the unguarded dereference and unfiltered-spread
+patterns. The database package evolved from a minimal prototype with import-time side effects, second-granularity
+timestamps, and no cleanup pass over exported surface. (16 findings: 1 critical / 4 medium / 11 low.)

+#### SDK-1 — MEDIUM — `addAgentStep` parses the response body before checking `response.ok`
+
+`database.ts:475` runs `await response.json()` **before** the `if (!response.ok)` check at `:476`. `startAgentRun`
+does it correctly (`:361` ok-check, then `:366` json). On an error response whose body is not JSON (HTML 502 page,
+empty body), `.json()` throws, the throw is caught by the outer catch (`:489`), and the failure is logged as
+`addAgentStep error` with no status code instead of `addAgentStep request failed`. Error classification and logs are
+wrong on the exact path that should be most informative. **Fix:** check `response.ok` first, then parse.
+
+#### SDK-2 — MEDIUM — `hasYieldedContent` omits tool-call/reasoning chunks → duplicate tool calls on ChatGPT OAuth fallback
+

+promptAiSdkStream({...skipChatGptOAuth})` at `:586`/`:630`) re-streams from scratch and the agent receives the
+tool-call chunks **twice** → duplicate tool execution. **Fix:** set `hasYieldedContent = true` on `tool-call` and
+`reasoning-delta` yields.
+
+#### SDK-3 — LOW — cost-override extraction block triplicated
+
+`llm.ts:719-734`, `:788-803`, `:858-873` — the `providerMetadata['savant-code'].usage → costOverrideDollars`
+extraction (~15 lines) is copy-pasted across `promptAiSdkStream`, `promptAiSdk`, and `promptAiSdkStructured`.
+Drift risk (FID-006-style shape changes propagate to only one copy). **Fix:** extract a
+`extractCostOverrideDollars(providerMetadata)` helper.

+#### SDK-4 — LOW — `isOAuthRateLimitError` / `isOAuthAuthError` are near-duplicates

+#### SDK-5 — LOW — `inputStream.pipeTo(...).catch(() => {})` swallows the upstream stream error
+
+`chatgpt-backend-fetch.ts:522` — the ChatGPT backend SSE source is piped into the transform with a silent catch.
+If the upstream stream errors mid-flight, whether the consumer sees the failure depends on the SDK's abort behavior
+(not guaranteed), and the rejection is unconditionally swallowed. **Fix:** forward the error to the readable side
+explicitly (or at minimum log it) so stream consumers fail loudly instead of hanging.
+
+#### SDK-6 — LOW — master-key exchange creates a new OpenRouter key on every process start
+
+`openrouter-key-resolver.ts:24-49` — with `OR_MASTER_KEY` set, every CLI process POSTs a new `savant-code-sdk` key
+(`limit: null`) with no dedup or cleanup, so keys accumulate on the user's OpenRouter dashboard. **Fix/verify:**
+document the exchange cadence or reuse an existing key; at minimum confirm this is intended.
+

+
+#### CMN-1 — MEDIUM — `coerceToArray` under `z.preprocess` drops `required` from the emitted JSON schema
+
+`tools/params/__tests__/coerce-to-array.test.ts:150` **fails at HEAD**: `z.toJSONSchema` of
+`z.preprocess(coerceToArray, z.array(z.string()))` omits `required: ['paths']` while the plain schema includes it
+(`- Expected - 3 / + Received + 0`). Tool schemas are emitted to the model via `z.toJSONSchema(inputSchema, {io:
+'input'})` at `packages/agent-runtime/src/run-agent-step.ts:117`, `packages/agent-runtime/src/tools/prompts.ts:42`,
+and `common/src/tools/compile-tool-definitions.ts:58` — so any param wrapped with `coerceToArray` (e.g.
+`ask-user.ts:19` `options`) tells the model the field is optional. The runtime coerce still tolerates string→array
+at the boundary, but model-facing requiredness is lost. **Fix:** investigate zod v4 preprocess schema emission and
+re-assert `required` (e.g. `.required()` or a schema-emission wrapper), then un-skip the test.
+
+#### CMN-2 — MEDIUM — `project-file-tree.test.ts` hangs on Windows
+
+`src/__tests__/project-file-tree.test.ts` — the second test (`scans regular project roots without a depth limit`,
+`:65`) hangs forever on Windows (exit 124). Root cause: `createFsWithFiles` (`:26-35`) walks
+`while (true) { ...; dir = path.dirname(dir) }` comparing against a POSIX root `/repo`; on win32
+`path.dirname('/repo')` returns `'\'` (drive root), so `dir === root` never matches and the loop spins forever. The
+first test passes because `os.homedir()` is a real Windows path. **Fix:** terminate the loop on a drive-root / path
+length bound, or normalize both sides with `path.resolve`.
+
+#### CMN-3 — LOW — Dead exports with zero callers
+
+Confirmed by repo-wide grep (only definitions + own tests reference them): `safeReplace` (`string.ts:291`),
+`parseToolCallXml` (`xml-parser.ts:8`), `MinHeap` (`min-heap.ts:4`), `splitData` (`split-data.ts:304`),
+`genAuthCode` (`credentials.ts:16`), `buildLogRows` (`log-ingest.ts:31`). ECHO Law 10/13 dead surface. **Fix:**
+remove or wire; keep `userSchema` (used by `sdk/src/credentials.ts:11`).

+#### CMN-4 — LOW — `safeReplace` doc says "replaces all occurrences" but replaces only the first

+`messages.ts:296-307` — recursion into nested objects with no depth bound. `llm.ts`'s `deepParseJson` has a
+depth-100 cap (FID-2026-0802-008 V4); this one does not. A deeply nested message content could overflow the stack.
+**Fix:** add a depth bound mirroring `deepParseJson`.
+
+#### CMN-9 — LOW — `withRetry` can `throw null` when `maxRetries` is 0
+
+`promise.ts:10-58` — the loop `for (attempt = 0; attempt < maxRetries; attempt++)` never runs when
+`maxRetries = 0`, then `throw lastError` throws `null`. **Fix:** guard `maxRetries` to `>= 1` or throw a real
+Error.
+
+#### CMN-10 — LOW — `promise.ts` `withTimeout` leaks its timer on the rejection path
+
+`promise.ts:40-52` — only the success branch (`promise.then(...)` → `clearTimeout`) clears the timer; if the
+wrapped promise rejects, the timeout still fires later (rejecting a settled promise is a no-op, but the timer holds
+the event loop / leaks). `mcp/client.ts:36-47` has a correct `.finally(() => clearTimeout(timer))` variant — the two
+implementations are inconsistent. **Fix:** use `.finally` like the MCP client.

+#### CMN-11 — LOW — saxy `_final` carries commented-out error branches

+
+The SDK inner layer is older than the outer layer and never received a dedicated hardening pass (FID-008 covered
+client/run/run-state only). Common/util is a shared grab-bag where dead code accumulates (six exports) and two
+pre-existing test defects (CMN-1 failure, CMN-2 Windows hang) have been silently masking the suite — CI has been
+green only because the failing/hanging files were never run to completion on Windows.
+
+## Proposed Solution (after approval — audit-only now)
+
+1. **Stage 1 — SDK correctness (SDK-1, SDK-2):** reorder `addAgentStep` ok-check before json parse; set
+   `hasYieldedContent` on tool-call/reasoning yields. Both with focused regression tests.
+2. **Stage 2 — SDK hygiene (SDK-3..7):** extract cost helper; unify OAuth error classifiers; forward/fix the
+   swallowed pipeTo error; document or dedup master-key exchange; drop the `agentTemplate!` assertion.
+3. **Stage 3 — common correctness (CMN-1, CMN-2):** fix coerceToArray schema `required` emission + test; fix the
+   Windows hang in `createFsWithFiles`. These two restore a fully green common suite.
+4. **Stage 4 — common hygiene (CMN-3..11):** remove/wire dead exports; fix safeReplace doc; type saxy predicate;
+   remove Law 6 `!`s; bound wellFormStrings recursion; guard withRetry; `.finally` in withTimeout; clear saxy

+1. FID-008 hardened the SDK's outer layer but not the inner execution engine — the duplicate-tool-call fallback

+   tracked per pass.
+2. A suite that hangs (CMN-2) and a suite that fails (CMN-1) can both go unnoticed when CI never completes the full
+   run on the primary platform — baseline "green" was only true for focused subsets.
+3. ECHO Law 6 `!` assertions and untyped `Function` parameters accumulate silently in shared util code; every
+   hardening pass should include a Law 6 sweep of the touched package.
