+# Codex LSP
+
+The MCP LSP server is not bundled in this LitCodex build. The plugin's `.mcp.json` declares no LSP server,
+and `components/lsp` is currently an inert Codex hook component: it parses `PostToolUse` and `PostCompact`
+payloads, then emits no diagnostics, definitions, references, symbols, rename edits, or cache-reset output.
+
+Read **[references/runtime-triage.md](references/runtime-triage.md)** before diagnosing any claim that a
+language server is installed, initialized, indexed, or available to Codex. The reference separates server
+installation, client transport, document lifecycle, feature capability, and Codex tool enrollment so an
+empty result is never misreported as a clean workspace.
+
+## What is actually wired
+
+- `components/lsp/dist/cli.js hook post-tool-use` — reads a Codex edit-hook payload and returns nothing.
+- `components/lsp/dist/cli.js hook post-compact` — reads a compact-hook payload and returns nothing.
+- `components/lsp/dist/cli.js mcp` — exits non-zero with an explicit unavailable message.
+
+Do not instruct users or agents to call LSP MCP tool names from LitCodex. If language-server evidence is
+required today, use the project's normal editor/CI/typecheck/linter commands, or a separately installed MCP
+server that the user has explicitly configured outside this package.
+
+## Required diagnostic packet
+
+For availability or diagnosis work, retain this packet internally and provide it in technical or audit
+mode:
+
+1. **surface inspected** — bundled component, explicit external client, or editor-only server;
+2. **capability state** — unavailable, executable-only, initialized, or feature-confirmed;
+3. **evidence** — exact probe, exit code, and raw result category;
+4. **fallback** — project command or scoped code-search method actually used;
+5. **limit** — what the fallback does not prove;
+6. **cleanup** — servers, ports, logs, and temporary configuration removed or intentionally retained.
+
+In reader mode, communicate the capability result, any material limitation, and the fallback or action
+the reader needs. Do not append exact probes, exit codes, raw categories, or routine cleanup merely to
+prove the diagnosis occurred.
+
+Never infer workspace-wide correctness from one file, one compiler invocation, or the presence of a server
+binary. Never modify editor or host configuration during a read-only diagnosis.
+
+## Relationship to setup notes
+
+`lsp-setup` remains useful as an advisory inventory of common language-server packages, but those notes do not
+activate an operational LitCodex diagnostics runtime. Treat any setup result as external/editor preparation
+only until this repo ships a real engine and corresponding tests.

+# Language-Server Runtime Triage
+
+Use this reference when a user expects symbol navigation, diagnostics, rename, hover, or workspace search
+from LitCodex. It distinguishes three different systems that are often called “LSP support”:
+
+1. a language-server executable installed on the machine;
+2. a client that starts the server and speaks the protocol;
+3. a Codex-visible tool or hook that exposes the result to the current session.
+
+Having only the first item does not make the feature operational. An editor can also have all three for
+itself while Codex has none.
+
+## Establish the capability boundary
+
+Check the repository and current installation before prescribing setup:

+Never describe a fallback as equivalent to a protocol result. Text search can find lexical references but
+cannot prove symbol identity. A clean compile can reject invalid code but cannot prove every editor
+diagnostic is absent.
+
+## Current LitCodex probe
+
+From the plugin source tree, the component’s public routes provide the authoritative product boundary:

+and stderr. An intentional no-output hook proves only that the placeholder accepted the event. It does not
+prove a server ran. A non-zero unavailable response from the `mcp` route is expected degraded-mode
+evidence in the current build.
+
+## External server diagnosis
+
+When the user has separately configured a client, diagnose it in protocol order.
+
+### 1. Executable and version
+
+Confirm the resolved binary, not merely the package-manager record:
+

+If a wrapper script resolves, inspect which runtime and package it actually launches. Version skew between
+an editor extension, global executable, project toolchain, and lockfile is a common cause of contradictory
+results.

+### 2. Root selection
+
+Determine the workspace root and the project marker the server expects. Monorepos frequently need one
+server per package or a root URI above several packages. A server started at the wrong root can initialize
+successfully while returning empty symbols and incomplete diagnostics.
+
+Capture:
+
+- canonical root path;
+- target document URI;
+- project/config file selected;
+- workspace folders sent by the client;
+- whether generated, vendored, and build directories are excluded.
+
+### 3. Initialization
+
+The client must send `initialize`, receive capabilities, then send `initialized`. Do not assume support for
+rename, workspace symbols, semantic tokens, or pull diagnostics. Read the returned capability object.
+
+If logs are available, verify request identifiers and response errors. A response for another document
+version is stale evidence.
+
+### 4. Document lifecycle
+
+For file-scoped features, verify that the client opened the document with the correct language id and
+version. After an edit, the client must send a matching change notification before requesting diagnostics
+or navigation. Save-only servers may require a write to disk.
+
+Check newline and position encodings when edits land in the wrong column. UTF-16 positions are common;
+byte offsets and Unicode scalar counts are not interchangeable.
+
+### 5. Result interpretation
+
+An empty result may mean:
+
+- the server returned a valid empty set;
+- the document was never opened;
+- indexing is incomplete;
+- the root is wrong;
+- the requested feature is unsupported;
+- the request was cancelled or timed out;
+- the response is stale;
+- the client filtered the result.
+
+Distinguish these with logs and capability data. Do not turn “empty” into “no problems” without that
+discrimination.
+
+## Safe rename fallback
+
+Without an operational rename provider, use a reversible manual sequence:
+
+1. identify the declaration and export boundary;
+2. search exact and transformed spellings;
+3. classify code, strings, configuration, generated files, and public documentation separately;
+4. capture compiler, typecheck, lint, and test baselines;
+5. edit one ownership region at a time;
+6. rerun the narrow check after each region;
+7. run the full relevant suite and the user-visible scenario;
+8. inspect the diff for accidental text replacements.
+
+Never run an unrestricted replacement across a repository based only on a spelling match.
+

+# LSP Setup
+
+**Automatic** LSP diagnostics are unavailable in this LitCodex build. The shipped `components/lsp` package is a
+Codex hook placeholder: it validates `PostToolUse` and `PostCompact` payload shapes, returns no diagnostics,
+and has no bundled MCP server. The plugin-level `.mcp.json` contains no LSP server registration.
+
+On-demand diagnostics are a different matter: `scripts/verify-lsp.ts` spawns the language server itself over
+stdio and needs no bundled engine, so a single-file check is real evidence.
+
+Use this skill to:
+
+- inventory which language-server executables a project may need;
+- point users to per-language install notes;
+- run a real on-demand roundtrip for one file once its server is installed;
+- explain that installing those executables still does not enable *automatic* post-edit diagnostics;
+- tell users to verify with their editor, normal CI, typecheck, or tests when they need broader evidence than
+  one file at a time.

+## PHASE 0 — LANGUAGE GATE (run first)
+
+Identify the language from the file extension, then read the matching reference before installing anything.
+References are package notes; `verify-lsp.ts` is what proves the server actually starts and answers.

+## WORKFLOW — detect → install outside LitCodex → verify outside LitCodex
+
+Before this workflow, record whether the user authorized installation or configuration changes. Detection
+and version probes are read-only. Package installation, editor changes, global PATH changes, and persistent
+server processes are mutations and require task authority.
+
+### 1. Detect advisory needs
+
+Scan the project to see which source-language extensions are present and whether common language-server
+executables already resolve on `PATH`:
+

+For each detected language it prints an advisory server id, the executable it would need, whether that
+executable is installed, a package install hint, and whether any known project config file mentions the id.
+This scan does not start a server and does not make LitCodex diagnostics operational.
+
+### 2. Install only if another surface needs it
+
+Open `references/<language>/README.md` and run the install command only when the user wants editor support,
+CI support, or preparation for a separately configured LSP client. Then confirm the executable resolves:
+

+Prefer the project’s pinned toolchain and package manager over a global install. Before using a reference
+command, reconcile it with the project lockfile, runtime version manager, operating system, CPU
+architecture, and existing editor configuration. Installation guidance ages quickly; treat package names
+as candidates until the package manager and the executable’s own version output confirm them.
+
+### 3. Configure with an explicit caveat
+
+The reference snippets show the historical JSON shape used by earlier LSP prototypes and by some external
+clients. In this build, writing `.codex/lsp-client.json` or `.litcodex/lsp.json` does **not** enable bundled
+LitCodex diagnostics. Keep config changes out of the user's repo unless the user has an external client that
+will read them.
+
+If you do write config for an external client, keep it project-scoped and minimal:

+### 4. Verify with a real roundtrip
+

+It resolves the server for the extension, spawns it, runs `initialize` → `initialized` → `didOpen` over stdio,
+and waits for `textDocument/publishDiagnostics` on the opened document.
+

+
+`--config=<path>` overrides the command, keyed by server id under `lsp`, the same shape `detect-lsp.ts`
+inventories; without it the project config files are tried, then the embedded table.
+
+This checks one file at a time. For whole-project health still run the project's normal typecheck, tests, or
+linter — this skill has no automatic post-edit diagnostics.
+
+### 5. Diagnose partial setup without guessing
+
+Classify failures at the first broken boundary:
+

+Do not reinstall at every failure. Reinstallation cannot repair a wrong workspace root, unsupported
+capability, stale document version, or missing Codex client surface.
+
+### 6. Cleanup and handoff
+
+Retain every persistent change in the internal handoff or requested audit record: installed package, modified
+config, PATH entry, downloaded runtime, and server process. In a reader-facing reply, surface a persistent
+change only when it materially affects the result or the user's next action; provide the full inventory when
+the user requests technical or audit detail. Remove temporary logs and stop probe processes. If editor restart
+or user login is required, say so explicitly and do not claim verification until the post-restart surface has
+been observed.
