"text": "Pour que votre dossier soit complet, vous devez joindre les documents suivants au formulaire de demande : - Copie d’un document d’identité officiel, délivré par une autorité publique, comportant vos nom, prénom, date et lieu de naissance, photographie et signature ainsi que l'identification de l'autorité qui a délivré le document (passeport, carte d’identité…) - Copie intégrale de votre acte de naissance, datée de moins de 3 mois, et non pas un extrait de cet acte. Vous devez faire votre demande auprès de la mairie de votre lieu de naissance. Attention : Il est recommandé de lire le livret d'information (figurant à la rubrique Notice) avant de remplir le formulaire. Après avoir vérifié que votre demande est recevable et complète, la CAPADD vous en accusera réception dans un délai de 2 mois. La CAPADD instruit ensuite la demande, recherche le tiers donneur et si ce dernier est identifié, sollicite son consentement. La CAPADD ne peut toutefois pas s’engager sur un délai de réponse, compte tenu de la difficulté potentielle à retrouver certains donneurs."
