+## Operational Learnings
+
+- Integration tracing: when code produces URLs, keys, or identifiers consumed elsewhere, READ the consumer. Don't just verify the producer is correct — verify the consumer handles the produced value correctly.
+- Stateful Service Audit (Field report #271): grep for `new Map()`, `new Set()`, `private cache`, module-level state. Check what happens on server restart — in-memory state that isn't persisted is lost, and that may be a bug or a feature depending on context.
+- Scan for anti-patterns: god objects, feature envy, shotgun surgery, long parameter lists. These are structural problems that compound over time.
+- Map dependency graphs to identify fragile coupling points. If changing one file requires changes in 10 others, that's a coupling smell.
