+A versioned library of agent capabilities, independent of any one generated project. Adapters
+compile each item outward into whatever the selected AI coding tool actually supports, and
+**skip the items their tool cannot express** rather than writing dead files.

+## Portability
+
+**Genuinely portable (24 of 29 items).** All 8 skills and all 10 commands work across every
+tool that has the matching surface. Skills degrade especially well: Claude Code loads one as a
+native skill, and every other tool receives the same markdown compiled into its rules or
+instructions file. `no-secrets` and `conventional-commit-check` are the most portable items in
+the registry — installed as native git hooks they protect humans and CI too, with no AI tool
+involved at all.
+
+**Claude-Code-only (5 items).** The three pure tool-lifecycle hooks —
+`guard-protected-paths`, `format-on-write`, `architecture-boundary-check` — plus the
+PreToolUse half of the two git hooks. No other adapter in the table can intercept a tool call
+before it executes, so nothing else can *block* a write.
+
+This asymmetry is the honest state of the ecosystem, and the reason `targets` exists.
+Cursor and Copilot have shipped hook-like features, but not with a
+`PreToolUse` + exit-code-2 contract, so claiming support would emit files that silently never
+run. When a tool gains a real equivalent, widen that item's `targets` — do not widen it
+in anticipation.
+
+Agents target `claude-code` and `github-copilot` only. Copilot's custom agents carry the same
+`name` / `description` / `tools` frontmatter and the same read-only reviewer shape.
+
+## Configuration
+
+No hook hardcodes a tunable value. Every one resolves through `hooks/_lib/`, in this order:
+
+1. **Env var** — `GROUNDPLAN_HOOK_<UPPER_SNAKE_KEY>` (arrays are comma-separated)
+2. **Repo config file** — `.agent/hooks.config.json`, overridable via `GROUNDPLAN_HOOK_CONFIG_PATH`
+3. **`hooks/_lib/defaults.json`** — the single typed default table
+
+`defaults.json` is the only place a default may be written. Both the PowerShell and bash
+implementations read it, so the two never drift.

+`boundaries` uses the rule syntax `"<pathRegex> !> <denyImportRegex> :: <why>"`, e.g.
+`"^src/domain/ !> src/(infrastructure|api)/ :: domain must not depend on outer layers"`.
+It defaults to `[]`, so the hook is inert until a repo declares its boundaries.
+
+### Windows and Linux
+
+Every hook ships as `hook.ps1` and `hook.sh` with identical behavior, because a repo generated
+on Windows may be cloned onto Linux CI. The bash side uses `jq` when present and falls back to
+a stdlib-only Python shim (`_lib/json_get.py`); with neither available the hooks exit 0 rather
+than failing the build. Both suites are verified to agree on the same fixtures.
+
+### How hooks get wired
+
+The `claude-code` adapter emits the `event` and `matcher` from each `hook.yaml` into
+`.claude/settings.json`, picking the interpreter available on the target platform:

+
+Exit codes are the contract: **2 blocks** the tool call and shows stderr to the model, **0
+allows** it. `format-on-write` never returns anything but 0 — a formatting failure is not a
+reason to fail a write.
+
+The two items with a `gitHook` key install additionally as real git hooks
+(`.git/hooks/commit-msg`, `.git/hooks/pre-commit`), where a non-zero exit rejects the commit.
+That path needs no AI tool at all, which is why those two are the registry's most portable
+items.
+
+## Adding a new item
+
+1. Create `registry/<kind>s/<id>/`. The directory name **is** the `id` — the loader resolves by it.
+2. Write the content file: `SKILL.md`, `command.md`, `agent.md`, or `hook.ps1` + `hook.sh`.
+3. Write `<kind>.yaml` against `RegistryItem`. Set `targets` to the tools that can *genuinely*
+   express the item; set `requiresCapability` to what it actually needs.
+4. Skills only: keep `SKILL.md` under ~150 lines and push depth into `references/`, linked
+   inline. Progressive disclosure is what keeps the item cheap to load.
+5. Hooks only: add any new tunable to `_lib/defaults.json` — never inline it in the hook.
+6. Add a row to the index above.
+
+### Rules
+
+- **Never ship a stub.** An empty file costs agent context on every single request and returns
+  nothing. If you cannot fill it, omit it.
+- **Declare `targets` honestly.** An item claimed for a tool that cannot run it becomes a dead
+  file in every repo that tool generates.
+- **No hardcoded config in logic** — no URLs, ports, model ids, paths, or magic limits. Values
+  resolve from `defaults.json` or from the repo's `AGENTS.md`.
+- **Commands that write files honor the two-phase contract**: Phase 1 resolves paths, reports
+  conflicts, prints the `Will create: / Will NOT touch: / Verification after apply:` manifest,
+  then stops for an explicit yes. Phase 2 writes, and every generated file carries a
+  `GENERATED by /<command> on <date> — review before commit` banner.
+- **Agents are read-only.** They report; the user decides.

+3. Extract every import and resolve its target module. Ignore third-party and standard-library imports — boundaries govern this repo's own modules.
+4. Compare each edge to the declared graph:
+   - undeclared edge → **violation**
+   - reverse of a declared edge → **inversion**; worse, because it fuses two modules into one
+   - edge that closes a cycle → **cycle**; always critical
+5. For each violation, check whether an ADR authorizes it. Authorized → report as `noted` with the ADR number, not as a failure.
+6. Check the leak that boundary lists miss: a type owned by an inner module appearing in an outer module's published signature. That couples them even when the import direction looks legal.

+Compare what was built against what `specs/NNN-slug/` says was agreed, and report the drift in both directions. **Do not edit code, the spec, or the task list.**

+## Mandatory reading
+1. `specs/NNN-slug/spec.md` — the WHAT and WHY. This is the contract.
+2. `specs/NNN-slug/tasks.md` — the agreed work and the files each task named.
+3. `specs/NNN-slug/plan.md` — when present, for the agreed approach.
+4. The files those tasks named, plus any file changed for this work that no task named.
+
+No `NNN-slug` given → list the directories under `specs/` and ask. Never pick one.
+
+## Process
+1. Build three lists from the spec: requirements, explicit non-goals, acceptance criteria.
+2. For each requirement, find the code satisfying it and cite `path:line`. Nothing → **unimplemented**.
+3. For each acceptance criterion, find the test or behavior proving it. Nothing → **unverified**, which is a different failure from unimplemented and is reported separately.
+4. For each task, compare its checkbox against reality:
+   - checked, but the named files show no corresponding change → **falsely closed**
+   - unchecked, but implemented → **stale list**
+5. Walk the changed files no task named. Classify each:
+   - serves a listed requirement → fine; note that the task list is incomplete
+   - serves nothing in the spec → **scope creep**
+   - does something the spec lists as a non-goal → **contradiction**, always critical
+6. Look for silent reinterpretation: behavior that satisfies the letter of a requirement while defeating its stated purpose. Quote the spec line and the code line side by side and let them argue.

+`FAIL` on ≥1 unimplemented requirement, contradiction, or falsely closed task. Scope creep alone is a `warning` — report it, but it does not fail the check by itself.

+## What NOT to do
+- Don't edit code, the spec, or `tasks.md`. Report only.
+- Don't check off tasks. Reporting the drift is the entire job.
+- Don't judge code quality — other reviewers do that.
+- Don't treat the spec as wrong when the code disagrees. The spec is the contract; the disagreement is the finding.
+- Don't infer requirements the spec never states.

+## Your only job
+Judge whether the tests added or changed actually pin the behavior they claim to. Report the gaps. **Do not edit or write any tests.**
+
+## The rule that defines this job
+
+**Never report a coverage percentage as a verdict.** Coverage counts which lines ran, not which behavior is protected — a test that calls a function and asserts nothing reaches 100%, and a suite of those proves exactly nothing. Once the number is the target it stops being a measure. Judge the assertions and the uncovered branches instead. Asked for a percentage, give the assertion analysis and explain why the number would mislead.

+## Mandatory reading
+1. `AGENTS.md` — the `Conventions` section for any declared testing rules, and `commands.test` so you can name the command that runs them.
+2. The test file(s) changed.
+3. The source file(s) under test.
+
+## Process
+1. Pair each test with the behavior it targets. A test you cannot pair with a behavior is itself a finding.
+2. Read every assertion and classify it:
+   - **Behavioral** — asserts the observable result or a state change a caller can see
+   - **Structural** — asserts a call happened on a mock; proves the code called something, not that it works
+   - **Tautological** — asserts a literal against itself, or re-derives the expected value using the code under test
+   - **Absent** — exercises code and asserts nothing, or only that it did not throw
+3. Enumerate the branches in the source under test — conditionals, early returns, catch blocks, default cases, boundary values. Mark each covered by a behavioral assertion, exercised without assertion, or untested.
+4. Check the error paths specifically. They are the branches that get skipped, and the ones that fail in production.
+5. Ask of each test: would it still pass if the implementation returned a constant, or if the feature were deleted? A test that survives that is not a test.
+6. Flag over-mocking. When everything a test touches is a mock, it asserts the shape of the test, not the behavior of the system.

+## What NOT to do
+- Don't edit or write tests. Report only.
+- Don't report a coverage percentage as the verdict. Ever.
+- Don't treat the number of tests as a quality signal — ten tautological tests are worth less than one behavioral one.
+- Don't demand tests for trivial accessors or generated code.
+- Don't run the suite. Name the declared test command and let the user run it.
+- Don't praise a green suite. Report what it fails to protect.
