+- fce6ee9: Fix machine-id allocation in the Cosmos cluster `RunnerStorage`. It derived the machine id from a 31-multiplier string hash of the runner address, so two distinct runners could hash to the same value mod 1024 — and since the machine id feeds the Snowflake generator mod 1024, those runners would emit colliding Snowflake ids (corrupting request/reply identity across the cluster).

+  Machine ids are now allocated from a single counter document via an atomic server-side `incr`, mirroring SQL's auto-increment `machine_id` primary key: unique across distinct runners and stable per address (a re-registering runner reuses its persisted id). Verified against a live Cosmos account.

+- be3300d: Fix correctness bugs in the Cosmos `WorkflowEngine`, found by running the adapter against a real Cosmos account. The Cosmos workflow engine now passes the same conformance suite as the in-memory and SQLite engines.

+  - **OCC conflicts were fatal.** `replaceExec` used `Effect.promise` + a `statusCode` check, but single-item Cosmos `replace` _throws_ on 409/412/404 (only `read` and batch ops surface the code), so every conflict became an unrecoverable defect and the `OptimisticConcurrencyException` catch was dead code. It now uses `Effect.tryPromise` and matches the thrown error (mirrors `ClusterCosmos`), so lease claims, completions, and interrupts lose gracefully under contention.

+  `ContextMapContainer.layer` once; every subsequent overlapping request received

+  (or overwritten) by another, and the finalizer was anchored to the first
+  request's scope.
